General FAQS

Is there a listing of all LSS Benefits in one spot that I can reference?

When will my benefits become effective?

Employees are eligible for medical, dental, vision, FSA, short-term disability, and/or life/AD&D coverage to begin the 1st of the month in which they will complete 90 days of full-time/weekend-option employment. Employees' long-term disability coverage does not begin until the first of the month following one consecutive year of full-time employment. Employees may enroll in the LSS Retirement Plan at any time they wish. For more info, please visit the “Enrollment and Coverage Dates” section of the “General Information” page.

How often are benefit premiums deducted from my paycheck?

All premiums for medical, dental, vision, life and voluntary insurance will be deducted from the participant’s payroll twice per month (24 times per year). Contributions for the Health Savings Account (HSA), Healthcare Flexible Spending Account (FSA), and/or Dependent Care FSA will be made from the participant's payroll every pay period (26 times per year). When can I change my benefit elections?

Everyone can make changes to all of their benefit elections during the Annual Enrollment period (typically held at LSS in November.) You may also change your coverage during the year only you experience a qualifying event such as a marriage, divorce, birth, adoption, loss of coverage through another plan, or status change to a benefits-eligible position. All changes must be made within 31 days of the qualifying life event and supporting documentation must be provided. Employees may make changes to their LSS Retirement Plan elections at any time they wish.

What do I need to do if a loved one passes away?

Is there any way for a Part-time employee to qualify for FT benefits such as medical and dental benefits?

Yes. In accordance with the PPACA (i.e. “ACA” or “Healthcare Reform”), if a part-time employee averages 30 or more paid hours during an employer’s designated “measurement period”, they become eligible for full-time benefits in the employer’s “stability period” that follows. Where can I view my current benefit elections?

You can view your current benefit elections inside Workday. Just login, go to the Menu, and then select the option labeled “Benefits”. Then select on the option to “View Benefit Elections”.

What if I am not eligible for LSS health benefits, but I’d like to explore coverage options I can privately carry?

Medical / Dental / Vision FAQS

I don’t need medical insurance through LSS because I have coverage elsewhere. Is there anything I need to do?

LSS offers eligible employees who choose not to elect LSS medical coverage an additional $43.33 of compensation per month (or $21.67 per pay period). To learn more about this program called the “Insurance Alternative”, please click here.

How do I find a participating provider?

You can find a healthcare provider that is in our covered network by visiting each insurance carrier’s website. Visit the benefit section of this LSS Benefits Website that you’re interested in and in each section, you’ll find a link to that insurance carrier’s website.

How can I obtain my Member ID cards?

Your insurance cards will arrive via US Mail to your home approximately 7-10 business days after your coverage becomes effective.  If you are needing immediate care prior to receiving your cards, please contact your local HR team. They will be able obtain temporary cards for you by working with the home office benefits team.   Once you do receive your ID cards in the mail, please register and create an account with your insurance carrier(s) to find participating providers, manage and view claims, request and view member ID cards, etc. (Anthem site or download the Anthem Sydney Health App; Delta Dental site; EyeMed site.

Wellness FAQS

Why do we need to complete the Wellness biometric steps to earn the lower priced medical premiums?

The most important way we can keep the prices of LSS’ medical plans low is through managing our health the best each of us can. The best way to manage our health as individuals is by being aware of our current health, i.e. knowing how we’re healthy and knowing what areas we may need to become healthier. This biometric screening process is how we learn more about our individual health and take steps to become healthier when necessary. By becoming healthier, the cost of our LSS medical insurance can stay lower, and it’s a win-win for everyone.

Can my family members also register in the Quest portal and complete a biometric screening?

A spouse of an employee can do so if they are covered as a dependent on one of our LSS medical plans. Unlike LSS employees, where all of us can register and complete a biometric screening (whether we’re full-time or not, and whether we’re enrolled in LSS medical insurance or not), spouses can only do so if they are covered on an LSS medical plan. Children cannot do any part of the program, even adult children.

I completed my Wellness biometric screening but I’m not seeing my “Rewards” in the Quest portal, why is that?

Since many employees have a personal account with Quest for their non-LSS medical records, sometimes employees accidentally visit the wrong Quest website instead of the LSS wellness portal associated with Quest. Make sure to verify that you are at this specific website to confirm that you are looking in the right place: my.questforhealth.com.
